I'm trying to set my eq, and am wondering what sound frequencies is each open string at? Low E is 40hz, right?
Also, what frequency is each string at it's respective 12th fret?

What about the other 3 strings? I checked that site, but they had no other info....

B0 (B string on 5 string bass) is 30.87Hz

E1 (E string on 4-string bass) is 41.2Hz.

12th fret is an octave above the open string, so multiply the open string frequency by two.

It's all there in that chart. A is 55 Hz, D is 73.42, G is 98. An octave doubles the frequency, so the 12th fret of each of those strings is 2x the frequency.
